When you purchase the three products, you can send in for the $5 mail-in-rebate using the form that’s printed at check-out. You will also receive another form that asks you to give your opinion and get $5.00! The form reads Take a Quick Survey and Get a Check! Log on at your convenience. It’s easy!. The form lists the website, along with a code and pin number to enter the survey. Upon completion of the survey, a $5.00 check will be mailed to you in 6-8 weeks! Offer expires 4/17/2011.
